Our Services:
Our clinic specializes in the management of the following allergies and conditions in adults and children:

Insurance Information:
	Fees for office visits and evaluation are usual and customary. We participate with most insurance companies. Please verify coverage with your insurance company prior to your appointment.
	We do submit to most insurance companies. Each plan has its own rules and regulations to which patients and physicians must adhere. Most insurance company policy manuals will give the required information, or you may call the customer service number located on your card.
	You must bring your current insurance card to your appointments. Co-payments required by your insurance company must be made at the time of service.
	We do not require a referral unless your insurance company requires a referral by your primary care physician. You are responsible for obtaining any referrals required by your insurance company.
